Students attending HCSD are required to be in compliance with health requirements by California Law. Please see below for grade specific requirements.
Transitional Kindergarten / Kindergarten Immunizations
All students enrolling in HCSD need to be up to date on their immunizations according to California Law. To ensure your child can start school, please make sure ALL forms and immunization records must be submitted to the school office BEFORE the first day of kindergarten.
Immunization Requirements for Transitional Kindergarten (TK)
According to California immunization law, students in the TK program are subject to the same immunization requirements as kindergarten. Since TK is considered the first year of a two-year Kindergarten program, the TB requirement to enter the TK program will be subject to the same requirement as kindergarten.

Oral Health Exam Requirement
Every kindergarten student is required to have a dental checkup by a licensed dentist. The oral Health Assessment form must be completed by a licensed dentist and submitted to the school by May 31st of your child's kindergarten year. Dental evaluations that occurred within the last 12 months may be used.
First Grade Health Exam
A health examination for school entry is no longer mandatory, but it is recommended to ensure students’ health and readiness for school.
TB (Tuberculosis) Risk Assessment for School Entry
A TB Risk Assessment must be completed by a health examiner before school entry, all new student admissions TK/Kindergarten through 12th grade.

7th Grade Immunization Requirement
All current and incoming 7th grade students are required by law to show proof of the Tdap vaccine, a booster for Tetanus, Diphtheria and Pertussis. This vaccine is administered on or after a child's 7th birthday. Beginning July 2019, two doses of Varicella will also be required. Please provide documentation to the Crocker school office.
